A Morselized Impaction Allograft Used for the Defect of Acetabulum in Revision Total Hip Arthroplasty - Minimum 10-Year Follow-Up -

Abstract
Purpose: This study is aimed to review the changes of the incorporation and allograft, not theloss of fixation, with the follow-up more than 10 years after using the morselized impaction allograftinto the acetabular bone defect in revision total hip arthroplasty.

Materials and Methods: This study involved 98 hips of 93 patients followed up minimal 10years (range, 10~20 years 2 months) in this clinic. The acetabular bone defects were classifiedwith the methods of AAOS and Paprosky. In all-cases, the morselized impaction allograft and themaximum diameter acetabular component were used. The final decision is compared and analyzedin the magnified anteroposterior and lateral radiographs postoperative, in the regular andfinal follow-up examination in this clinic. This study is intended to review on the new bones formation,the change of the radiolucent zone between the allograft and cup, and the boundarychange of the morselized allograft and the formation of the trabecular. 97 cases were fixed wellexcept 1 case.

Results: Radiographs postoperative and the final follow-up examination were compared andthen, additionally, the interim follow-up examination were compared step by step. If the interimfollow-up examination cannot be available, it is only compared with radiographs postoperativeand the final follow-up. There were 3 cases showing the radiolucent zones between the host boneand the allograft and 12 cases of the periacetabular radiolucent zones, both being less than 2 mm.Excluding 15 cases, there was the formation of trabecular between the host bone and the allograft.And there was the remodeling of medial side graft through a process of acetabular boneabsorption.

Conclusion: The rapid deposition of new bone and the formation of trabecular were gottenfrom the bone defect in the weight bearing portion by using the morselized allograft into theacetabular bone defect in revision total hip arthroplasty. Medial side protrusion graft when themedial side bone defect was severe, undergo the remodeling through a process of bone absorption.Therefore, it is considered that the remodeling of the allograft is reformed along the patternof the stress change after the weight bearing.
